Just a reminder that our school will have its annual Flu Shot clinic this Thursday, October 6 at 9am. This optional service is provided by Weber Morgan Health. The Flu Shot Clinic is also open to any community member as well. If you need additional information or another information packet, the site is http://www.webermorganhealth.org/flu_immunizations.php
If you are interested in your child receiving his/her flu shot here at the school, please make sure the document is filled out, signed and money sent if applicable.
If you have a 6th grade student, he/she can receive the rest of the immunization shots required for entry into 7th grade during this same clinic. A parent may claim an exemption to immunization for medical, religious, or personal reasons, as allowed by Section 53A-11-302 of the Utah Statutory Code.
